You have signed an application form for membership of Pendle Hill Football Club Incorporated an affiliated member club of Granville District Soccer Football Association (GDSFA) and of Football NSW.  This Privacy Policy Statement relates to the collection, maintenance, use and disclosure of personal information about you and provided by you and in all future communications between you, Pendle Hill Football Club Incorporated, GDSFA and Football NSW directly or indirectly.You have signed an application form for membership of Pendle Hill Football Club Incorporated an affiliated member club of Granville District Soccer Football Association (GDSFA) and of Football NSW.  This Privacy Policy Statement relates to the collection, maintenance, use and disclosure of personal information about you and provided by you and in all future communications between you, Pendle Hill Football Club Incorporated, GDSFA and Football NSW directly or indirectly.

1. Collecting organisation and what information we collect

  • In its capacity as an affiliated member of Granville District Soccer Football Association (GDSFA) and Football NSW, the Pendle Hill Football Club Incorporated collects personal information. Such information may include name, date of birth, address, telephone number, e-mail address and grading status.

  • GDSFA and Football NSW and the Pendle Hill Football Club Incorporated maintain a member’s register, which may include personal information as summarised in the dot point above.

2. Purpose of collecting information

The primary purpose of collection of personal information is the maintenance of registers of members to facilitate registration of the Pendle Hill Football Club Incorporated with Granville District Soccer Football Association and Football NSW, for insurance purposes, for players, for coaches, for managers, for communication with team members and team officials of Pendle Hill Football Club Incorporated (either by telephone, mail, web site or e-mail), for communication between team members and team officials to facilitate the provision of registration details, etc.  Additionally, the information may be used to manage and administer the activities of GDSFA, Football NSW and Pendle Hill Football Club Incorporated to include payment of fees and charges as levied.  We take reasonable steps to ensure that whenever we collect, use or disclose personal information it is accurate, complete and up to date.  The Pendle Hill Football Club Incorporated will take all reasonable steps to destroy or permanently de-identify all personal information if it is no longer required for any purpose for which the information may be used or disclosed under National Policy guidelines.

3. Consequence of not providing information

If you do not provide complete and accurate information, the Pendle Hill Football Club Incorporated, GDSFA and Football NSW may not be able to effectively maintain their members’ register. If we do not get the personal information we need, we, or any other involved organisation, may not be able to provide the appropriate services and insurance required to enable a team member or team official to carry out duties associated with playing Football.

Except for the purposes stated in this Statement and GDSFA's and Football NSW's Privacy Policy your personal details will not be provided or disclosed to any other organisation or company without your explicit consent, unless required by law.

4. Disclosure of information to other organisations

Your personal information may be disclosed to organisations outside the Pendle Hill Football Club Incorporated, GDSFA and or Football NSW and this may include internal or outsource service providers who complete operational tasks and manage the necessary components of our day to day operations to include information technology services, market research, debt recovery requirements, customer inquiries, recruitment inquiries, insurance requirements, match payments, regulatory bodies, marketing and marketing services.  This could include, recruitment inquiries, insurance requirements, match payments, regulatory bodies including the Department of Fair Trading, N.S.W. Police services, medical services and the Australian Taxation Office.

Your personal information may also be disclosed to Management Committee members and or Office Bearers of the Pendle Hill Football Club Incorporated, GDSFA and or Football NSW for the purpose of up-loading information into accounting systems managed by the Treasurer, mailing of annual reports by the Secretary, notices of meetings and clearance requirements for member application between other associations.

5. Access to information

Prior to supplying any information to the Pendle Hill Football Club Incorporated, GDSFA and or Football NSW, you should familiarise yourself with the Privacy Policy Principles of GDSFA, Football NSW and or the Pendle Hill Football Club Incorporated.  If you complete and sign any forms or notices of advice that are provided to the Pendle Hill Football Club Incorporated, GDSFA or Football NSW, it will be deemed that your signature is acknowledgment that you agree to supply the information.

You can gain access to your personal information by writing to the secretary of the Pendle Hill Football Club Incorporated, GDSFA and/or the secretary of Football NSW or advise us at any time if the information held is inaccurate, incomplete or has changed.

6. Photographs and posting of images.

The Pendle Hill Football Club Incorporated may at its discretion use photographs taken at Football matches at which you play and/or are involved and post them on its web site to promote Pendle Hill Football Club Incorporated or to record your successful participation in local district football.

The Pendle Hill Football Club Incorporated has established a website http://pendlehillfc.com for the provision of internet content for its members and the worldwide web at large.  Unless advised to the contrary, permission is deemed to be received for the purpose of posting uploaded images either individually or as a group. This is to advertise and recognise, either, social events, honorary appointments or special awards received by individual members or teams of players from the PHFC.  Members who do not wish to have their image appear on the website, should either contact the Webmaster or the PHFC Secretary. Written consent by letter or email or by way of the application form is required for photos to be displayed on the website by members under the age of 18 years.
